Physical Exam Event

In collaboration with IU Health, the West Lafayette Athletics Department will be holding its annual physical exam night for athletes on the evening of Thursday, May 7th from 5:45 pm – 8:00 pm. Registration and payment are required by Tuesday, May 5th and is limited to the first 130 student-athletes. In addition, IU Health is also offering an opportunity for athletes to have their sports physicals administered on Saturday, May 9th at IU Health Arnett Orthopedics from 8:00 am – 12:00 pm. Registration and payment are required by Thursday, May 7th. Space is limited. These events are not mandatory as you may have your child see his/her primary care provider. It is encouraged to keep an annual wellness visit with your child's provider. As a reminder, a pre-participation physical exam dated 4/1/2026 or after must be on file with the school in order for athletes to participate in summer or pre-season workouts. Unless your child is in a spring sport that continues through the post season, all physicals are considered expired at the conclusion of the school year. Please see additional information below. This is an opportunity to update medical history, insurance coverage, etc. E-signatures are required annually from both parents and athletes. IHSAA Pre-Participation Physical Exam Packet Emergency Medicines: Carry/Self-Administer Permission Form If your child requires an emergency medicine such as an inhaler or EpiPen, a permission to carry and self-administer form must be on file with the school each year: Please have the prescribing physician complete the form as well as a parent/guardian and return to the school.
