In order to participate in athletics at West Lafayette Jr. Sr. High School, each family (student-athlete) will be required to create a Final Forms account. You can create your Final Forms account by clicking the link below for the parent playbook and then following the instructions. The athletics department will be utilizing this platform to digitize previous forms that were required to participate. We will also have some additional forms and information on the site. Moving forward, this will be an efficient way for the department and coaches to communicate with parents and student-athletes.
If you turned in an IHSAA physical dated after April 1st, 2019, for the 2019-20 school year, that physical will be valid for the 20-21 school year. You do not have to obtain a new physical for the upcoming school year if you had a physical for the 2019-20 school year on file with the athletics department. However, you will be required to submit a paper copy of the “IHSAA Health History Update Questionnaire and Consent and Release Certificate“. This document can be printed and returned to Athletics Director: Joel Strode, or Athletics Administrative Assistant: Kathy Slauter. If your student-athlete needs an updated physical exam, that form can be printed from Final Forms.
